HARROW CROSSROADS WITH OUTREACH
NIGHT
SERVICE - CRITERIA
Who is eligible?
Carers:-
·
who live in the London Borough of Harrow
·
who provide a substantial amount of care
for someone with a disability or illness, normally in their own home
·
who may or may not be already in receipt
of Harrow Crossroads’ service
Priority will be
given to those Carers whose sleep is disturbed and own health suffering, or may
suffer, as a result of their overnight caring.
How is the service accessed?
Referrals can be made by -
·
Carer or person needing
care
·
A relative or friend
·
Social Workers
·
GPs and other health
workers
·
Other statutory or
voluntary organisations, etc.
The Service
Following
a referral -
·
an assessment visit is arranged – a
thorough assessment of needs, a
general risk assessment, and a moving
and handling risk assessment is carried out
·
after
consultation with the Carer and Cared for, a full Care Plan of needs and duties
to be carried
out by the Care Support Worker is drawn up,
and a copy kept in the home.
·
a service is arranged to suit individual
circumstances – usually a 10 hour slot between
9.00 p.m. and 9.0 a.m.
·
It may not be
possible to provide a service if substantial medical care is required
